The Role

We are seeking a compassionate, motivated and resourceful Community Worker to join our growing team. This is a mobile, community-based position requiring travel throughout the Sydney metropolitan area. Services are delivered across participants’ homes, community settings, schools, programs and other service locations rather than from one fixed workplace. Regular travel between different suburbs and service locations is an inherent requirement of the role.

 

This is a professional community services role combining community development, disability support, needs and risk assessments, family support, advocacy, referrals, program development, community engagement and participant capacity building.

 

The Community Worker will work directly with individuals and families while also helping The Inclusion Spot identify community needs, develop inclusive programs and strengthen connections between participants, families, community organisations, Government and Non-Government services, allied health professionals and other local resources.

Task & responsibilities

Key Responsibilities

  • Plan, develop, coordinate and deliver community programs based on needs, that foster inclusion, social connection and participation.
  • Consult individuals and families to identify their needs, strengths, barriers, circumstances and social or community support requirements.
  • Identify gaps in local community supports and contribute ideas for new services and programs
  • Conduct risk assessment and develop individualised care plans that are in line with participant goals and needs
  • Support participants to develop independence, communication, social, daily-living and community participation skills in line with their goals. Assist with transport, and community access such as shopping.
  • Facilitate community engagement through group activities, events and programs.
  • Support families by providing information/resources to understand and navigate available community, disability, health, education and social services. Make referrals/link with appropriate supports where needed.
  • Advocate with and on behalf of participants and families to help address barriers to services, community participation and social inclusion.
  • Assist participants with community access, social participation, skill development and daily activities.
  • Contribute to outreach and community engagement activities that increase inclusion, social connection and participation for people with disability and their families.
  • Assist with the preparation of program proposals, reports, resource requests and submissions relating to new or existing community initiatives where required.
  • Maintain accurate participant records, assessments, progress notes, program documentation and reports in accordance with organisational and regulatory requirements.
  • Work collaboratively as part of a multi-disciplinary team with occupational therapists, speech pathologists, physiotherapists, social workers, behaviour support practitioners and other professionals.

Qualification & experience

About You 

You are passionate, compassionate and empathetic, and thrive in dynamic environments. You must be confident working with individuals, families, a multi-disciplinary team and community organisations, but also have an ability to work independently. You have strong communication, assessment, planning, problem-solving and organisational skills and a demonstrated commitment to inclusion and person-centred practice.

 

Essential Qualifications and Experience

  • An Australian Qualifications Framework (AQF) Diploma or higher qualification in Community Services, Community Development, Human Services, Social Work or a closely related field, or a recognised overseas equivalent.
  • A minimum of one year of relevant full-time paid work experience in community work, community services, family support, disability services, community development or a closely related professional field.
  • Demonstrated experience assessing individual or family needs and developing appropriate support, education or community-based strategies.
  • Demonstrated ability to develop, coordinate or contribute to community, educational,  support or capacity-building programs.
  • Strong written and verbal communication skills, including the ability to prepare professional case notes, reports and program documentation.
  • Experience working with people with disability and their families is highly regarded.
  • Valid First Aid & CPR certification, NDIS Worker Screening Check, and Working with Children Check (or willingness to obtain).
  • A current unrestricted Australian driver’s licence and reliable vehicle.
  • Access to a reliable and appropriately insured motor vehicle.
  • Ability and willingness to travel regularly throughout the Sydney metropolitan area to participant homes, community locations, schools, service providers and other work-related locations.
