Job Description
As a Community Support Worker at DY COMMUNITY GROUP PTY LTD, you will work with a dedicated team committed to improving the wellbeing, independence, and quality of life of individuals and families within the community. Duties & Responsibility
- Interview and assess clients to identify social, emotional, disability-related and community-support needs
- Assess the nature and extent of client difficulties affecting independence, safety and participation
- Plan, develop and implement individualised support and community participation strategies
- Identify and coordinate appropriate health, welfare, housing, employment, training and community services for clients
- Refer clients to external agencies and follow up to confirm access to required supports
- Liaise with community groups, welfare agencies, allied health professionals, healthcare providers and government services
- Coordinate communication between clients, families/carers and service providers to ensure continuity of support
- Monitor and document client progress, service delivery outcomes and significant changes in circumstances
- Identify service gaps, risks or safeguarding concerns and escalate appropriately
- Support clients and families to understand available choices, advocate for their needs, and build capacity for independent decision-making
- Promote awareness of community resources and services among clients and stakeholders
- Contribute to case reviews, reporting and documentation in line with organisational and regulatory requirements
Qualifications & Requirements
- AQF associate degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2).
- Atleast 1-year relevant work experience is required
- Strong communication, interpersonal and problem-solving skills.
- A passionate, caring and empathetic personality is a must.

Reporting To
The position reports to the Managing Director of DY COMMUNITY GROUP PTY LTD.
Working Hours
Full-time position, 38 hours per week.

Business operations include client intake, needs assessment, support planning, service coordination, direct support, staff allocation, case documentation, stakeholder liaison, progress monitoring, referrals, risk management and ongoing review of services.
The combined Community Support Worker, Support Coordinator and Support Worker functions contribute to the daily operation of DY COMMUNITY GROUP PTY LTD by connecting assessment, planning, coordination and direct service delivery. The employee assists the business to maintain continuity of support, respond to changing client needs, coordinate staff and external services, document client outcomes, manage service issues and promote client independence and community participation.
These operations are supported by the employee's background in disability services, mental health, aged care, client advocacy, case coordination, crisis intervention, multidisciplinary collaboration and community services practice.
