Social Worker (Heart to Heart Disability Support)

We are seeking compassionate, reliable, and dedicated Disability Support Workers to join our NDIS disability support team on a full-time or part-time basis. The role involves providing person-centred support to NDIS participants, including participants with complex support needs and behaviours of concern, while promoting their dignity, independence, safety and individual goals. Duties may include personal care, assistance with daily living, medication support where appropriately trained, community access, appointments and social activities. Working hours are flexible and may include morning, afternoon, evening, weekend and overnight shifts, including sleepover shifts. Successful applicants must be reliable, patient, professional and able to remain calm and respectful when supporting clients with complex needs. Previous disability or community support experience is highly regarded, and applicants must be willing to work as part of a team and follow individual support plans and workplace procedures.
