About The Role
We are seeking Community Workers (8 Positions) to assess individual and family support needs, develop and review support programs, coordinate referrals and advocate for access to appropriate services. You will work with participants, families, carers and community organisations to address barriers to social participation and help people achieve their goals. The role includes client assessment, case management, service coordination, community engagement and accurate reporting.
Key responsibilities
• Interview participants and consult with families and carers to identify social, emotional and practical needs, strengths and goals.
• Assess support needs and develop individualised support plans in consultation with participants and relevant professionals.
• Provide case management, monitor progress and review support arrangements as needs and circumstances change.
• Identify barriers to participation and coordinate referrals to health, housing, education, employment and welfare services.
• Provide information about available services and advocate for participants’ rights, informed choices and access to support.
• Plan and facilitate community participation activities and support programs that promote inclusion and independence.
• Liaise with support coordinators, health professionals, government agencies and community organisations to coordinate services.
• Maintain accurate case notes, assessments, progress reports and service documentation while protecting confidentiality.
• Identify safeguarding concerns and risks, report incidents and escalate urgent needs through organisational procedures.
• Participate in case reviews and team meetings and contribute to service improvement and community needs assessment.
• Follow organisational policies, workplace health and safety procedures and relevant NDIS quality and safeguarding requirements.
Essential requirements
• Cert III or Diploma or higher qualification in Community Services, Social Work or a closely related field or work experience equivalent
• At least one year of relevant experience in community services, disability services or a related role.
• Demonstrated ability to assess client needs, develop support plans and provide case management or coordinated referrals.
• Knowledge of the NDIS framework, person-centred practice, safeguarding, confidentiality and professional boundaries.
• Strong communication, advocacy, documentation and organisational skills.
• Ability to work respectfully with people from diverse cultural backgrounds and with varied support needs.
• Current National Police Clearance and NDIS Worker Screening clearance, or willingness to obtain required clearances before commencement.
• Current First Aid and CPR certification, or willingness to obtain before commencement.
• Valid driver’s licence recognised in Western Australia and willingness to travel for work.
• Working with Children Check where required for the duties.
Desirable experience
• Experience supporting people with intellectual disability or autism and working collaboratively with their families and carers.
• Knowledge of local community services and experience facilitating social inclusion programs.
• Experience using electronic client records and preparing progress reports.
Employment conditions
This is a full-time role of 38 hours per week. The advertised base salary is $75,000–$95,000 per annum, depending on qualifications and relevant experience, plus superannuation. Applicable entitlements, allowances and penalty rates will be provided in accordance with the relevant award or industrial instrument. Travel between service locations and flexible working hours may be required to meet participant needs.
