Triage and assess complex health needs Coordinate care across services Deliver expert clinical interventions Support hospital avoidance and early discharge Build resilience through restorative care Partner with multidisciplinary and community teams Manage short term case coordination where needed Work across Integrated Care sites as required About You Bring your clinical expertise, initiative and commitment to quality care.
