Key tasks and primary responsibilities

  1. Family and Liaison Advocacy 
  • Act as a primacy point of contact for families, providing emotional, practical and social support. 
  • Empower families by helping them navigate aged care services, funding models, and available community resources. 
  • Advocate for resident's' rights, choices and independence to ensure their voice is central to their care. 
  • Manage challenging or sensitive situations with high levels of empathy, professionalism, and effective problem-solving.

   2.  Emotional and Social Support 

  • Build meaningful relationships with residents, providing companionship and emotional support.
  • Engage residents in activities and social interactions to promote mental well-being. 
  • Respect residents' individuality, preferences, and cultural needs in all interactions.

   3.  Assistance with Daily Living 

  • Help residents maintain a clean and safe living environment by assisting with light housekeeping tasks. 
  • Make beds, tidy rooms, and ensure residents have access to necessary personal items.
  • Monitor and report changes in residents' routines, preferences, or behaviours to supervisors. 

   4. Documentation and Communication 

  • Record and report care activities, observations, and any changes in residents' conditions to the RN or Team Leader. 
  • Maintain accurate and timely entries in care documentation systems. 
  • Participate in handovers to ensure continuity of care between shifts. 

   5. Compliance with Policies and Procedures  

  • Adhere to infection control and hygiene protocols, including proper use of personal protective equipment (PPE).
  • Follow workplace health and safety guidelines to ensure the safety of residents, staff, and visitors. 
  • Respect residents' privacy, confidentiality and dignity in all care activities.

   6. Collaboration with Team Members 

  • Work effectively with other team members, including nurses, allied health professionals, and kitchen staff, to meet resident's needs. 
  • Attend team meetings and training sessions to stay informed about policies and procedures. 
  • Provide feedback on resident's care plans to improve outcomes. 

   7.  Emergency and incident Response 

  • Respond promptly and appropriately to emergencies, including falls, behavioural incidents, or medical emergencies. 
  • Report incidents and complete documentation as required by the organisation.
  • Participate in drills and maintain readiness for evacuation procedures or other emergencies.

  • Ability to work as part of a team and follow directions from supervisors
