Achieve Something Great Join the Diabetes Case Conferencing team, a collaborative and innovative team dedicated to improving diabetes care and outcomes across South Western Sydney. Contribute to expanding access to specialist diabetes support by working directly with general practices to deliver Diabetes Case Conferencing and integrated models of care. Develop meaningful partnerships with General Practitioners, Practice Nurses and multidisciplinary healthcare teams to support evidence-based diabetes management in the community. Work within a flexible service model, delivering care and education through telehealth and onsite visits to general practices across South Western Sydney. Play a key role in improving diabetes care for people living in South Western Sydney. Are You the Right Fit? The candidate must be a Credentialled Diabetes Educator, or a Registered Nurse actively working towards obtaining Credentialled Diabetes Educator status and make a meaningful difference in the lives of people living with diabetes. Apply your diabetes education expertise to support healthcare professionals and empower patients to achieve better health outcomes across primary and specialist care settings. Communicate diabetes management concepts clearly and effectively to General Practitioners, Practice Nurses and patients, adapting your approach to face-to-face, telehealth and virtual environments. Work as part of the Diabetes, Obesity and Metabolism Translational Research Unit (DOMTRU), a team committed to improving diabetes outcomes through innovation, education, collaboration, and translational research, including a willingness to participate in and support research projects that advance diabetes care and service delivery. Use a range of health service systems, such as Microsoft Office, electronic medical records and Consultmed to coordinate patient bookings, manage referrals and support the seamless delivery of diabetes services across multiple care settings. Selection Criteria To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below. Current Authority to Practice as a registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA). Are you a Credentialled Diabetes Educator, or a Registered Nurse actively working towards obtaining Credentialled Diabetes Educator? Please indicate your current status. Describe your experience in diabetes education and how you have supported patients and healthcare professionals to improve diabetes management and health outcomes. This role requires the ability to communicate complex diabetes management concepts to General Practitioners, Practice Nurses and patients across face-to-face, telehealth and virtual settings. Describe a situation where you successfully adapted your communication style to meet the needs of different audiences. Provide an example of how you have worked collaboratively within a multidisciplinary team to achieve a positive outcome for patients, service delivery or quality improvement.
