About the Position
VHCC is seeking a Community Worker to join its Box Hill team in a newly created full-time role. The Community Worker will work directly with NDIS participants and their families to assess their needs, develop and implement individualised plans, and connect them with the services and funded supports that move them towards their goals. Working within VHCC’s multidisciplinary service model and liaising closely with the NDIS and external providers, the role is central to ensuring that each participant’s plan is personalised, properly coordinated, delivered within budget and regularly reviewed, so that participants build their capacity for independent living and receive consistent, high-quality support.
Key Responsibilities
Interview participants and their families to assess their needs and determine the nature and extent of their difficulties.
Prepare, manage and implement individualised NDIS plans, ensuring each plan is personalised, practical and actionable.
Provide direct, person-centred assistance to NDIS participants and their families in line with their goals, needs and preferences.
Advise participants and their families on the NDIS process, including their rights and responsibilities.
Assist participants to work towards the goals outlined in their NDIS plans, monitoring progress and adjusting support strategies as needed.
Provide emotional support and maintain clear, effective communication with participants, families and stakeholders.
Liaise with and report to the NDIS on participants’ progress and plans.
Connect participants with appropriate services and funded supports, ensuring their needs are met effectively.
Ensure participant plans are implemented in accordance with the allocated budget and NDIS funding guidelines.
Collaborate with internal teams and external services to maximise outcomes for participants through a coordinated and integrated support system.
Assist participants to coordinate and implement supports that build their capacity for independent living.
Requirements
Australian citizens and permanent residents are encouraged to apply.
Diploma in Community Services.
Minimum 1 year of relevant experience.
Sound working knowledge of the NDIS, including plan implementation, funding categories, budget management and reporting requirements.
Ability to conduct needs assessments, prepare clear case notes and progress reports, and maintain accurate participant records in line with privacy and NDIS Practice Standards.
Understanding of person-centred, trauma-informed and rights-based practice.
Excellent communication and interpersonal skills, with the ability to build trust with participants, families and service providers from diverse backgrounds.
Strong organisational, time management and problem-solving skills, with the ability to manage a varied caseload.
Empathy, resilience and sound judgement when supporting participants through complex or sensitive circumstances.
Current driver licence and willingness to travel to participants’ homes and community settings across greater Melbourne.
