Achieve Something Great This role offers a diverse range of assessment and intervention opportunities across telehealth and outpatient clinics within South Western Sydney LHD, with a focus on delivering evidence-based, non-surgical care for people with osteoarthritis. Join an innovative, district-wide Osteoarthritis Chronic Care Program (OACCP) focused on improving patient outcomes and reducing the need for joint replacement. Lead comprehensive assessment, education and pre-surgical optimisation to support patients to delay or avoid surgery and achieve better outcomes when surgery is required. Play a key role in the expansion of services across South Western Sydney, including support for patients on orthopaedic waitlists and improved access to care at Bankstown Hospital. Collaborate within a multidisciplinary team of allied health, nursing, medical and administrative professionals to implement the ACI OACCP Model of Care and the Osteoarthritis Clinical Care Standard. Contribute to a funded service improvement project through June 2028, helping shape model-of-care development, service evaluation, patient experience and health system outcomes. Are You the Right Fit? We are seeking a clinician who is passionate about delivering person-centred, evidence-based care and can build strong relationships while contributing to service development and innovation. Passion for improving outcomes for people with osteoarthritis and those awaiting joint replacement surgery. Excellent communication, interpersonal and active listening skills, with the ability to engage effectively with patients, carers, multidisciplinary teams and stakeholders. Strong collaborative skills and the ability to work effectively within a multidisciplinary team. Demonstrated leadership capability, with the ability to influence practice and support service development and innovative models of care. Strong attention to detail and commitment to high-quality care. Selection Criteria To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below. Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA). Relevant post-registration qualifications and at least 3 years' experience working in the clinical area of the employees specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation. Demonstrated autonomy and critical thinking in decision making, professional knowledge and judgment in providing evidenced based complex care requiring advanced clinical skills. Demonstrated ability to effectively communicate and engage with patients-consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs while providing compassionate quality care.
