- Assisting participants with access and planning processes.
- Providing information on disability, community, and mainstream supports.
- Developing and monitoring community connection plans.
- Conducting participant check-ins and supporting plan changes where required.
- Building relationships with community organisations and service providers.
- Supporting community engagement activities and local service delivery.
- Maintaining accurate records and completing administrative tasks.
- Experience in disability, community services, health, human services, or a related field.
- Strong stakeholder engagement and customer service skills.
- Experience working with Aboriginal and Torres Strait Islander communities.
- Ability to manage competing priorities and solve problems effectively.
- Strong written and verbal communication skills.
- Experience working independently and as part of a team.
- Proficiency using Microsoft Office and client management systems.
- Willingness to obtain an NDIS Worker Screening Check and Working with Children Check.
- Knowledge of the disability sector and person-centred service delivery.
- Experience in regional, rural, or remote communities.
- Australian Citizenship required.
- APS4 and APS5 opportunities available.
- Hybrid working arrangements.
- Travel within local communities may be required.
- Driving licence preferred.
- Community and participant home visits may form part of the role.
